Sometimes the trigger is trauma.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Commotio cordis is a rare event in which a sudden blunt impact to the chest occurs at a vulnerable instant in the cardiac cycle and triggers a dangerous heart rhythm, most commonly ventricular fibrillation. The athlete&#8217;s heart may have been structurally normal before the impact.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How can a chest impact stop the heart?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The heart&#8217;s electrical system follows a tightly controlled sequence. During a very small period in the cardiac cycle, the ventricles are especially vulnerable to disruption. If a blunt object strikes the chest over the heart at the wrong moment and with sufficient force, the impact can trigger ventricular fibrillation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Instead of producing coordinated pumping contractions, the ventricles electrically and mechanically disorganize. Circulation stops. The result is sudden cardiac arrest.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Which sports are associated with commotio cordis?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Reported cases have historically involved sports in which fast-moving objects can strike the chest, including baseball, softball, lacrosse, and hockey. Physical collisions can also produce the required chest impact. Cases occur disproportionately among boys and young men, although the condition is not limited to them.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The American Heart Association describes commotio cordis as very rare and reports roughly 10 to 20 new U.S. cases annually, while noting the possibility of underrecognition.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The athlete may collapse immediately<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The classic emergency presentation is sudden collapse after a chest impact, though a person can occasionally remain conscious or mobile for several seconds before falling. If the athlete becomes unresponsive and is not breathing normally, responders should treat the situation as cardiac arrest rather than waiting to determine whether the impact looked severe enough.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The response is immediate activation of EMS, CPR, and AED use.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Survival has improved<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Historically, commotio cordis had a poor survival rate. Registry research has documented substantial improvement over time as awareness, CPR, and defibrillator access improved. The American Heart Association currently reports survival above 50% and emphasizes immediate CPR and AED use.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is a powerful lesson in emergency systems: a condition can remain medically dangerous while outcomes improve because recognition and response improve.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Can commotio cordis be prevented?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">No strategy can eliminate all risk from contact and projectile sports. Safer playing techniques can reduce avoidable chest impacts. Age-appropriate softer balls may be considered in some youth settings. Protective equipment has important injury-prevention roles, but it should never be treated as a substitute for emergency readiness.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Is commotio cordis the same as having heart disease?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">No. Commotio cordis is a mechanically triggered arrhythmic event. However, a survivor should still receive comprehensive cardiac evaluation to exclude an underlying cardiovascular disorder that could have contributed to the arrest or been mistaken for commotio cordis.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The RTH takeaway<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Commotio cordis is rare enough that many athletes will never encounter a case. That is not a reason to ignore it. It is a reason to use preparedness measures that also protect against every other form of sudden cardiac arrest: trained responders, rapid recognition, accessible AEDs, and a practiced emergency plan.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Recognize. Respond.
